Emergency Surgery: Vitreoretinal Scleral Fixation of IOL

Emergency eye care

The eye is the most sensitive organ in the body that is exposed to the outside world. Though it is protected from all sides by a bony box known as the orbit of the eye, front of the eye is open to the outside world and hence vulnerable to injury.

Road traffic accidents are one of the most common ways that an eye can get injured. It is always advisable to use safety measures like the use of helmet on two-wheelers and seat belts in cars.
In cases where these measures are not used, the eye can get seriously injured. The video shows one such case or a young individual who had fall from two-wheelers. His glasses had broken and one-piece went in back of the eye known as Vitreous cavity. He had undergone suturing of the corneal injury 2-3 weeks before retinal surgery.

During the retinal surgery very thick vitreous hemorrhage can be seen. This hemorrhage is cleared and glass piece stuck inside removed. Glass foreign bodies inside the eye are very difficult to remove since they are slippery and can’t be firmly held with devices available. The video demonstrates the use of claw forceps which were extensively used to remove pellets from the eyes of victims in Kashmir after the 2016 unrest.

This has been developed by Dr. Maneesh Bapaye of Dr. Bapaye Hospital and it can be used to remove objects such as glass pieces or stones or shotgun pellets. At the end of surgery intraocular lens was implanted by scleral fixation technique. A very severe injury is treated successfully with a single surgery
